Non Medical Practitioner (Prescriber)

Job Introduction

HMP Peterborough are currently recruiting for a Non Medical Practitioner (Prescriber) to support, train and advise Clinical Nurse Managers and the wider healthcare team in chronic disease management. 

HMP/YOI Peterborough is the country’s only purpose-built prison for both male and female prisoners (kept separate at all times), including a 12-place mother and baby unit. We are committed to offering education and development programmes to provide residents with the skills required to enter employment on release, so reducing the chances of further offending. Providing creative outlets, such as workshop activities, helps to equip those in custody for when they are released. We use ‘Restorative Approaches’ to bring people in conflict into dialogue. Using a skilled and structured process, those who have been harmed have an opportunity to be heard and those that have caused the harm are held to account for what they have done. We have strong links with the community outside through a variety of partnerships, including our outreach activity Outside Links’ offering support and advice to both residents and their families.

Role Responsibility

  • Non-medical prescriber within Primary Care setting
  • Work closely with GPs, the SJS Lead Pharmacist and all other Healthcare staff to ensure positive outcomes for patients.
  • Assess patients presenting with acute illness or for review of chronic disease conditions.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies
  • Work with patients  in order to support compliance with and adherence to prescribed treatments
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Implement and participate in vaccination and immunisation programmes for adults
  • Provide information and advice on prescribed or over-the-counter medication, side-effects and interactions

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
  • Communicate with and support patients who are receiving ‘bad news’
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
  • Attend and contribute to practice and primary care team meetings Delivering a quality service
  • Produce accurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures
  • Deliver care according to NICE and National guidance and evidence-based care
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation
  • Work with practice management to ensure sufficient staff of appropriate ability, quality and skill-mix are available to meet current and future service delivery, that selection and recruitment processes are effective and that equality of treatment of the team incorporates quality HR principles and processes
  • Discuss, highlight and work with the team to create opportunities to improve patient care
  • Manage and lead on the delivery of specifically identified services or projects as agreed with the practice management team
  • Collate, analyse and present clinical data and information to the team
  • Learning and development Undertake mentorship for more junior staff, assessing competence against set standards

Experience

  • Minimum 5 years post registration experience
  • primary nursing experience
  • Nurse-led management of minor illness, minor ailments and injuries
  • Nurse-led triage including telephone triage
  • Compiling protocols and clinical guidelines
  • Leadership in quality initiatives such as clinical benchmarking
  • Audit
  • Extended and Supplementary Nurse prescribing

Qualifications

  • Registered first level nurse Non Medical Prescriber
  • Relevant nursing/health degree
  • Clinical supervision training and experience

Package Description

Salary £43,772,81 per annum

The role will require working a variation of shifts that average 37.5 hours per week
